These neuropeptides play a pivotal role in modulating pain, eliciting pleasure, and maintaining homeostasis within the central nervous system.

TBI triggers a cascade of pathological processes, including shearing and deformation of neural tissues, inflammation, oxidative stress, and neuronal apoptosis, significantly impacting neuropeptide production. The hypothalamic-pituitary-adrenal (HPA) axis may become dysregulated, impairing the hypothalamus's ability to synthesize endorphins from proopiomelanocortin (POMC). Additionally, the neuroinflammatory response activates glial cells and releases pro-inflammatory cytokines, further inhibiting endorphin synthesis and release.

The psychophysiological repercussions of TBI—such as chronic pain, anxiety, and depression—can perpetuate a vicious cycle, wherein decreased endorphin levels exacerbate symptoms, impairing the patient’s quality of life and emotional well-being.

In summary, the interplay of mechanical trauma, neuroinflammation, and hormonal dysregulation can precipitate an endorphin deficiency, underscoring the importance of recognizing ENDEF not merely as a symptom but as a significant consequence of trauma's impact on neurophysiological systems.
